			<content:encoded><![CDATA[<p>One of the most common phone calls I receive in the office is when someone&#8217;s mother or father is admitted to the hospital. In this time of crisis, answers are not easy to come by.</p>
<p>How does their health insurance work? What does Medicare pay for? Once the parent is discharged, what happens, where do they go, how is it paid for, what are our options? What do we do if mom or dad is going to have to go to a nursing home? How do we pay for it?</p>
<p>This confusion is expected as the senior health care system can be a very confusing and overwhelming process. The first thing to do is to understand the basis for today&#8217;s system.</p>
<p>In 1983, Congress created the Prospective Payment System. This is important because when a person 65 or older is admitted to a hospital, he is assigned only one of 473 Diagnostic Related Groups (DRG&#8217;s). This is important because Medicare compensates the hospital a flat dollar amount for the DRG assigned to the patient.</p>
<p>Let me give you an example. Say that my father is admitted to the hospital with lung problems and the DRG is four days. If my father is discharged in three days, then the hospital makes one day of profit. If my father is discharged in five days then the hospital loses money and cannot bill the patient for the one extra day.</p>
<p>Back in the good old days, I remember when my grandfather was in the hospital and the nurse asked him if he felt well enough to go home because if he didn&#8217;t, he could stay a few extra days until he felt better.</p>
<p>Today, it is all about the money. Once a patient is no longer getting better or worse, in other words, is deemed to be &#8220;stable&#8221;, then the patient is discharged either to home or a Medicare certified nursing home or rehab facility.</p>
<p>In order for Medicare to pay for rehab care the patient must have been in the hospital for three consecutive days (72 hours). Then, no later than thirty days after discharge from the hospital, be admitted to a Medicare certified nursing facility.</p>
<p>If these criteria are met, then for 2010, day&#8217;s one through twenty in the rehab facility are paid for 100% by Medicare. For days twenty one through one hundred, your co pay is for this year is $137.00 per day.</p>
<p>From day 101 and beyond, regardless of your condition, you are responsible for all of the facility costs.</p>
<p>Keep in mind, that in order for this reimbursement schedule to happen, you must either be getting better or getting worse. Like the hospital, once you are deemed to be stable, you come off the Medicare reimbursement schedule and must pay for all costs.</p>
<p>In California, most patients will come off of Medicare reimbursement around week three and must begin private paying from this point forward. The business office will advise you when this is expected to take place.</p>
<p>If the facility has long-term care beds, then the patient may be able to stay in the same facility. But if the facility is strictly short-term care or rehab, then the patient must find another facility or go home.</p>
<p>How does the patient&#8217;s health insurance fit into this? It all depends on what type of plan that the senior patient is on. Is it a Medicare supplement plan or PPO, or is it a Medicare Advantage plan like an HMO?</p>
<p>Medicare supplement insurance, also called Medigap, is private health insurance designed to supplement Medicare. A premium is paid for this coverage which is age rated.</p>
<p>There are twelve standardized Medigap plans, A through L. In most states, you can go to any doctor or hospital that accepts Medicare without pre-authorization. Under plans C through J, days one through twenty are completely paid for by Medicare. For days twenty one through one hundred, the Medicare co-pay for 2010 is $137.00 which is covered by the Medigap policy. From day one hundred one and beyond, the patient is responsible for the full cost.</p>
<p>For Medicare Advantage plans such as an HMO like Secure Horizons, SCAN and Kaiser, the patients may have a co-pay from day eleven of $100. It is best check the benefits booklet or call the customer service department.</p>
<p>If someone goes to a facility without going to the hospital first, then you must private pay from day one.</p>
<p>Once the patient comes off Medicare reimbursement, if qualified, Medi-Cal will help to pay for the nursing home costs. If going to the facility directly from home, then, if qualified, Medi-Cal may help to pay for the nursing home costs from day one.</p>

Due to the explosive coverage on the story, much of which implied that breastfeeding is public is indecent, many moms are now confused about what their actual rights are by law to breastfeed when they are out in the world.</p>
<p>Clearly feeding your child is a human right that insures the safety of your infant&#8217;s health. And of course a feeding baby is better that a hungry, screaming baby, especially in constricted spaces like airplanes. So why all the fuss?</p>
<p>These attitudes derive from the taboo of breastfeeding in this society, not rational thought into the matter. Ironically, we see images of breasts every day on television and in movies. And of course popular women&#8217;s styles always involve the power of cleavage. This is not a breast-phobic society by any stretch of the imagination.</p>
<p>Breastfeeding and the law</p>
<p>The state laws do not reflect the moral center of gravity in the United States. Laws about breastfeeding in public differ from state to state, but overall, most breastfeeding laws are designed to protect the mother&#8217;s right, not restrict it. In fact, 21 states have laws that explicitly exempt public breastfeeding from public indecency charges. This fact reveals that popular opinion about breastfeeding in public is mixed, otherwise the law would not have been needed. Here&#8217;s a good resource for breastfeeding laws state by state.</p>
<p>Breastfeeding and Advertising</p>
<p>The truth is a little weirder. People are not offended by a bare breast, but actually by a bare breast that is not sexually depicted. In other words, if a breast is bared without being sexy, but instead to be used for its intended purpose to feed a child, then we&#8217;ve got indecent exposure on our hands!</p>
<p>Some might argue that media images are actually half-covered breasts, but remember that a nursing child has got the &#8220;offending&#8221; part of the breast covered too. There is simply no way to suggest that breastfeeding in public is indecent by our own standards of publicly viewed advertising.</p>
<p>Advertisers rely on sexual imagery to sell their products, and part of the taboo on breastfeeding has to do with the way it strips the sexuality from the Almighty Rack. The real boob is the man who buys pornography but is offended by breastfeeding &#8211; revealing the split in many men&#8217;s psyches between their sexual desires and mothering realities.</p>
<p>But let&#8217;s not cry over spilt milk, boys. When angry protesters gathered in Texas after the Delta incident last fall, they knew they would cause the biggest stir by breast-feeding in public, in mass. They call themselves &#8220;Lactivists.&#8221;</p>
<p>Texas police tried to disband the group of breast-feeding mothers by threatening them with arrest for public exposure and indecency. The lactivists had done their homework and quickly bared a copy of the law that allows them to breastfeed in public. Similar protests took place in Oregon, Georgia and Vermont.</p>
<p>So, review the breastfeeding laws in your home state. At the very least, the law may stipulate that a mother is &#8220;discrete&#8221; about nursing in public. This simply means using a blanket to cover up, or feeding in an unpopulated public area. Perhaps even the call to be discrete is ridiculous, but it&#8217;s always good to know the law.</p>
<p>The bottom line is, don&#8217;t be made to feel ashamed for breastfeeding in public. It&#8217;s the right thing to do, always. The only thing being exposed is our society&#8217;s double standard.</p>

<p>Try not to schedule the party for a time when you&#8217;ve got other things going on. It may seem like a child&#8217;s party is nothing major, but planning it too close to other big events can add tremendously to your stress level. If possible, steer clear of dates that are within a few days of a big work presentation or business conference, another special occasion, or even a family vacation.</p>

<p>Charge batteries; empty memory cards; and change tapes before the party. Having a video recorder or digital camera run out of batteries or memory during a party often leads to heated &#8220;blaming&#8221; fights. I&#8217;m not sure that any mother in the world would trade her children for a beautiful newborn baby doll, but I&#8217;m sure there are times when you had that thought &#8211; huh? Let&#8217;s face it &#8211; kids can be real trouble. They&#8217;re noisy, can be cranky and they can be a real mess. But they do grow up, eventually, and leave you to go off to claim their part of the world.</p>
<p>Why is all the excitement over newborn baby dolls these days? And why are they becoming so popular, even though some of these realistic dolls can cost upwards of several hundred dollars! I think I may have some answers for you, and you may be surprised at what other people are doing &#8211; and why.</p>
<p>There is a movement afoot in the doll industry called &#8220;reborners.&#8221; These folks create incredibly realistic dolls as a hobby, and some do it to make a living. Many of these &#8220;reborn&#8221; dolls are sold on exclusive websites or even eBay for hundreds of dollars.</p>
<p>Some of these craftspeople will even create a custom doll for you &#8211; just send then a photo of the baby you want to &#8220;replicate&#8221; and they will make it happen for you.</p>
<p>The very newborn baby dolls that are so popular now are called a &#8220;reborn&#8221; doll. Everything about these incredible creations are designed to duplicate the real thing &#8211; right down to skin color and texture, hair, dimensions and weight. In fact, these newborn baby dolls are so realistic looking you may stop and check the &#8220;baby&#8217;s&#8221; breathing if you see one in a crib. They are that true-to-life! Of course, don&#8217;t be alarmed, you won&#8217;t see a single breath, but their ability to duplicate the real thing will take your breath away!</p>

These remedies will not only save you money but may actually resolve your problem. Allow me to tell you about my mom&#8217;s favorite four remedies.</p>
<p>My mother&#8217;s cure for our upset stomachs, our insect bites and our small burns was raw honey. This is not some old wife&#8217;s tale but an actual resolve. Do you know that honey has bacteria killing properties? Well it does. For your upset stomach, you mix equal parts of honey and vinegar (about one teaspoon each) and simply add it to your water or your juice. Now when you have a mild cough you should drink it straight. When dealing with a burn, dress your bandage with it. Note; children under one should never consume raw honey as it has spores that produce bacteria that may lead to botulism. Needless to say this may not be a good remedy for you folks with severe bee allergies; caution is the suggestion here.</p>
<p>Got any ginger in that kitchen cabinet of yours? Well it is a good remedy for nausea and motion sickness. The best way to use it is in a tea or ground ginger or fresh chewy slices. This is generally safe in low doses but obviously if you have any allergic reactions see you doctor.</p>
<p>While you are looking in that kitchen cabinet do you see your cinnamon? Well it is a great remedy for your bad breath, how about that! Also it has been found that cinnamon has some antiseptic qualities and may even kill bacteria. So look to it for help in nausea and germs. Use it by making tea mixed with one half a teaspoon of cinnamon powder in a cup of boiling water. Now for your bad breath, just mix it with cool water and use it as your mouthwash.</p>
<p>And my mother&#8217;s favorite &#8211; baking soda. Great for your mild gastritis between your meals, it really works! Your baking soda has antacid qualities and may neutralize your heartburn, try it as I believe you will like it. You may get relief very quickly, possibly in thirty to forty- five seconds by merely stirring one half spoon of it into four ounces of warm water and simply sipping it. Who knows this may make you feel better.</p>
<p>Well guys these are my mom&#8217;s favorite four. She used them on my brothers and I and they really did work. Never overdo the amounts I have recommended and if you are on medication, talk to your doctor before using any of them. And of course should you get any complication consult your doctor immediately. And as always you must eat well to be well.</p>

			<content:encoded><![CDATA[<p>As a pediatrician, I care about more than just your children&#8217;s physical health. Mental and emotional well-being are equally important. Sadly, the greatest preventable threat to your child&#8217;s physical health also threatens their mental and emotional health. I&#8217;m talking, of course, about childhood obesity.</p>
<p>Studies cited by the American Academy of Pediatrics (AAP.org) find that &#8220;obese children have lower self esteem and self confidence than their thinner peers.&#8221; No big surprise. Kids haven&#8217;t learned to separate their sense of &#8220;self&#8221; from their body image, so when they perceive themselves to be less attractive than other kids, they believe they have less value, too. Unfortunately, this belief is reinforced by everything they see on TV and in magazines, which trumpet beauty as the most important asset a person can have. We know better, but our kids don&#8217;t.</p>
<p>The other kids your children are around don&#8217;t know better, either, which means that overweight children get more than their fair share of teasing, and they have a harder time making friends. Remember how important your friends were when you were a child? Imagine getting through one hour on the playground without friends to play with, watching the other kids from a distance.</p>
<p>Inside the classroom, your child&#8217;s schoolwork can suffer because of a lack of self esteem. Then poor grades make them feel even worse about themselves creating a downward spiral.</p>
<p><strong>What you can do</strong></p>
<p>The first and most important piece of advice is: Don&#8217;t become part of the problem. Don&#8217;t try to tease your children into changing their eating habits. Making fun of your child&#8217;s weight is not the way to help him or her lose a few pounds. Instead of positioning yourself as the enemy, become part of your child&#8217;s support system. Reinforce everything you love about this special person, every unique and wonderful quality. Let your home be a haven from the stresses of the world.</p>
<p>At the same time, do encourage healthier eating by setting a positive example. Be honest with yourself. Do you prepare healthy meals for your kids? Do you make healthy choices when grocery shopping?</p>
<p>Start by creating a more active life for your family by getting involved in fun activities with your child. Even taking a brisk walk around the neighborhood can be fun, and it can give you and your kids a chance to chat about your day.</p>
<p>Being fit and healthy physically increases your child&#8217;s chance to be happy. Next time you reach for an unhealthy snack, think about the cost to your child&#8217;s long-term happiness.</p>
